Description

DESCRIPTION:

We are seeking an experienced mental health professional who is committed to making a difference in the lives of vulnerable individuals. This role involves providing flexible supports and individualized skills, training and education to Health in Supportive Housing clients in their home environment. In alignment with each Health in Supportive Housing Client’s Individual Recovery Plan, support services may include enhancing skill acquisition, promoting independence, assisting with daily living and self-management skills, providing education and training to support informed lifestyle decisions, and facilitating meaningful engagement with community resources. The successful candidate will work collaboratively with clients, community partners, and health professionals to facilitate long-term well-being and community integration.
SALARY RANGE: Grid 34 – HEU – Community Subsector
EMPLOYMENT CATEGORY: Full-Time, Permanent
NUMBER OF VACANCIES: 1 (one) vacancy
HOURS OF WORK: Sunday to Thursday 11:00-19:00

QUALIFICATIONS

  • A diploma in Community Social Services OR an equivalent combination of relevant education, training, and experience, including lived experience.
  • Minimum of 2 years of recent related experience, including lived or living experience with mental health and substance use.
  • Strong understanding of mental illness, recovery-oriented approaches, and harm reduction principles
  • Familiarity with community resources and service navigation
  • Effective written and verbal communication skills.
  • Ability to work both independently and collaboratively within a team
  • Valid Class 5 driver’s license and access to a reliable vehicle.
  • Physical ability to perform the duties of the position.
  • Current First Aid and CPR certification. 
  • Naloxone Training.
  • Provincial Violence Prevention Training.
  • Successful clearance of a criminal record check.

Providing Client-Centered Support and Recovery Planning:

  • Empowering clients to manage daily living activities and make informed lifestyle decisions through education, training, and ongoing support.
  • Identifying and connecting clients with services and community resources that align with their needs and interests.
  • Collaborating with Burnaby Mental Health case managers to support client-directed goals.
  • Developing and implementing Recovery Plan Reviews in collaboration with clients and case managers.
  • Monitoring progress and evaluating outcomes of Recovery Plans and Wellness Recovery Action Plans (WRAP).
  • Assisting with self-management strategies, including medication management, symptom improvement, relapse prevention, and crisis response.
  • Providing specialized support to promote independence and reduce reliance on the formal mental health and substance use system.

Enhancing Community Engagement and Supporting Client Independence:

  • Accompanying clients to community appointments and assisting with follow-ups.
  • Providing transportation using a personal vehicle when required while ensuring a safe and supportive experience.
  • Responding to client crises and reporting incidents in alignment with established protocols.
  • Facilitating program admissions and orientations, incorporating family and natural supports where appropriate.

Ensuring Compliance with Ethical and Professional Standards:

  • Maintaining accurate and timely documentation in accordance with organizational and ethical guidelines.
  • Adhering to confidentiality, privacy, and professional boundaries while providing client care.
  • Participating in ongoing professional development and training as required.
  • Performing Additional Responsibilities as Needed.
