Description:

OVERVIEW

We are seeking a dedicated and compassionate Nurse Assessor to join our healthcare team. In this role, you will be responsible for conducting thorough assessments of patients’ health needs and providing recommendations for care plans. Your expertise will be crucial in ensuring that patients receive the appropriate support and services tailored to their individual requirements. This position requires strong clinical skills, excellent communication abilities, and a commitment to patient-centred care.
You’ll be working with NHS North Central London Integrated Care Board (NCL ICB) they are a statutory body responsible for planning and allocating health and care resources to improve the lives of people in our five boroughs: Barnet, Camden, Enfield, Haringey, and Islington.
The local population is large and diverse, with a wide range of health and care needs that are evolving all the time. They are committed to improving the health and experience of their patients and residents, working with partners across the North Central London Integrated Care System (ICS) - including Councils, providers, GPs and voluntary and community organisations - to plan and deliver high quality, joined-up and accessible health and care services that put people first.
The post holder will assist in the implementation and delivery of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (revised 2018) and be able to apply it within the context of the assessment role.

The FNC Clinical Assessor will be responsible for:

  • working within the NHS Continuing Healthcare Service to support the Clinical Assessors to establish and maintain robust systems to implement current policy and guidance relating to NHS Continuing Healthcare, Funded Nursing Care and Retrospective reviews and claims.
  • using experiential knowledge and clinical skills to ensure that assessments and reviews for FNC and outcomes are determined in a consistent manner to ensure effective application and interpretation of the National Framework for NHS Continuing Health Care and NHS Funded Nursing Care (revised October 2018)
  • working with relevant health professionals, CHC colleagues, business support staff, senior leads and other support staff within the CHC team to ensure delivery of key outcomes of the service.
  • supporting with the ‘End of Life’ care and reviews, following the national guidance for clients ‘Fast Tracked’ through the CHC system.
  • facilitating smooth patient flow across all aspects of the patient’s journey including liaison with primary care, secondary care, community venues and specialist services.
  • Shadow Complex Care practitioners to gain the skills to undertake CHC assessments.

SKILLS

  • A minimum of 6 months recent experience in a similar role
  • Registered Nurse (RN) qualification with a valid NMC registration.
  • Strong clinical assessment skills with the ability to identify patient needs effectively.
  • Excellent communication skills, both verbal and written, to interact with patients, families, and healthcare professionals.
  • Ability to work independently as well as part of a team in a fast-paced environment.
  • Proficient in using electronic health records (EHR) systems for documentation purposes.
  • Empathy and compassion towards patients, demonstrating a commitment to high-quality care.

Responsibilities:

RESPONSIBILITIES

  • Conduct comprehensive health assessments for patients in various settings, including home visits and healthcare facilities and nursing homes. complete a minimum of 3-4 FNC reviews per day.
  • Evaluate patients’ medical histories, current health conditions, and social circumstances to inform care planning.
  • Collaborate with multidisciplinary teams to develop and implement personalised care plans.
  • Provide education and support to patients and their families regarding health management and available resources.
  • Maintain accurate documentation of assessments, recommendations, and follow-up actions in accordance with organisational policies.
  • Stay updated on best practices in nursing assessment and patient care.
