Description:
This doctor will be part of the multidisciplinary team managing acute and chronic patients as part of the take and looking after in patients on our Liver ward. The post holder will work alongside 3 other foundation year 1 doctors. All FY1s will generally be ward based during the ‘normal’ working day and expected to deliver the daily medical care of all the patients currently under the care of the Hepatology team, which includes general medical patients in addition to liver patients. There are daily consultant ward rounds. When on-call for general medicine, F1s will be involved with the clerking of patients being admitted and the on-going care of newly admitted patients. The FY1 doctor is responsible with other staff for the ward care of patients, contributing to the acute medical take and the maintenance of the patient’s medical record. When ward work allows there are opportunities to attend a variety of specialist outpatient clinics with the consultants. FY1 doctors are expected to attend the departmental meetings including Radiology and Cirrhosis MDTs, inpatient MDT’s, Histology meetings, Research in progress presentations, and the weekly Grand Round. In addition, they will be expected to attend the structured teaching programmes provided.
The FY1 doctor will be responsible for such other specific clinical duties as allocated by consultants, including performing other duties in occasional emergencies and unforeseen circumstances.
The post involves working as part of a team on St Marys Hospital site with exposure to and opportunities to learn from:
- Ward based care of Hepatology and General Medical inpatients on the Acute Medical wards and Douglas ward.
- The opportunity to learn practical procedures including ascitic tap, abdominal paracentesis, lumbar puncture, joint aspiration and central venous catheter insertion.
- Assessing patients admitted acutely under the acute medical take and Hepatology teams and under supervision, being responsible for the quality and continuity of clinical care of these patients in and out of hours.
- The opportunity to attend Specialist Hepatology clinics including Pre-Transplant, General Hepatology, Cirrhosis, NAFLD, HCC, Viral Hepatitis, Family liver clinic, Autoimmune, Enhanced Supportive Care and Alcohol related liver disease clinics.
- The opportunity to attend endoscopy lists.
- Working with the multi professional team including nurses, interventional radiologists, surgeons, dieticians, physiotherapists, occupational therapists, advanced nurse practitioners, social workers and discharge coordinators.
- Contributing to the governance and audit structures within the department.
- Contributing to the education and training of other team members including allied health professionals.

3.1 The work of the department
The Dept. of Hepatology comprises 14 Consultants of whom 5 do acute General Adult Medical on-call. There is a strong academic component to the Team (which has five ICSM Professors) Academic Clinical Fellows, 4-5 SpRs and several Clinical Research Fellows. Our department’s special interests include Cirrhosis, Viral Hepatitis, Liver Cancer, Metabolic dysfunction associated steatotic liver disease Disease and Alcohol related Hepatitis with active research programmes in all these areas, including a Clinical Trials Unit.
St Mary’s is a major teaching hospital, as part of Imperial College London, and medical students are often attached to this department, leading to teaching opportunities for the F1 if desired.
