Looking for something different from traditional general practice?
Would you like to become a member of our Primary Care Directorate clinical team providing innovative care to our patient population as part of an outstanding CQC organisation and HSJ Trust of the Year?
We are looking to appoint a highly motivated and committed General Practitioner to join our clinical team. This is a pivotal post in the delivery of quality and innovative primary care services. We are a friendly and well organised team with a forward thinking approach.
We are looking for a commitment of 4 sessions per week (plus CPD), to be undertaken on Wednesdays and Fridays primarily based at Moorcroft Medical Centre and Moss Green Surgery, but offering cross cover where needed to support other bases within our integrated practice portfolio.
We are a high achieving, patient centred practice and have implemented a care model to improve patient experience, providing urgent and routine appointments. Standard clinical sessions and the provision of administration time for non-patient facing work within the session is routinely included.
Excellent clinical support from our extended clinical team, including ANPs and UCPs.
Positive promotion of learning and CPD supported by weekly protected learning events along with the benefits of being part of a larger Trust.
You will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
Team work is an essential part of the role, working as part of our senior clinical team to provide quality care to our patients as well as supporting the wider practice core clinical workload.

Key Duties/Responsibilities
In accordance with the Job Plan as agreed, the post-holder will make themselves available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
Screening patients for disease risk factors and early signs of illness.
In consultation with patients and in line with current Practice disease management protocols, developing care plans for health.
Providing counselling and health education.
Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.
Recording clear and contemporaneous consultation notes to agreed standards using the practice electronic clinical system.
Collecting data for audit purposes
Compiling and issuing computer-generated acute and repeat prescriptions
Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate.
In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care
