Royal Primary Care Brooklyn is seeking an experienced healthcare assistant with an interest in frailty to support our multi-disciplinary team of professionals – including working alongside our colleagues in the ARCH PCN (Alfreton, Ripley, Crich and Heanor) and other stakeholders in the community.
This is a developing role within our practice team providing care to vulnerable, frail and housebound patients. This role will also involve working alongside GPs and Advanced Clinical Practitioners aligned to care homes, supporting with HCA tasks, the weekly ward round and annual long-term condition reviews as required.

Royal Primary Care have a team of over 170 people dedicated to our patients, working across 3 sectors covering Chesterfield, Clay Cross and Heanor. Our Nursing team are essential to ensuring that we always provide an outstanding level of nursing care to our patients.
You will work collaboratively as part of our multidisciplinary (Heanor) team of clinicians to meet the needs of our patients, delivering the highest standards of patient care. You will undertake a diverse range of assessments and provide the appropriate care / treatment according to patient needs, striving to improve standards of care and enhancing nursing services within the practice.
Depending on experience, contribution to the monitoring of Long-Term Chronic Disease management of our patients will also form a large part of the role. Knowledge of the GMS contract e.g. The Quality and Outcomes Framework will be valuable to the post.
