Description:

Patient Care Coordinator REPORTS To: Reception Supervisor HOURS: 26.25 hours per week, alternative weekly shift pattern. Monday to Friday 8am - 1pm, 1pm - 6.30pm. Additional hours available to cover staff holidays, sickness and other adhoc duties. Job Summary: Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way.
Provide general assistance to the Practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone Job Responsibilities: Administration To have a thorough knowledge of all Practice procedures. To work in accordance of written protocols Photocopy as requested General administration duties as necessary Reception Greeting and receiving patients and visitors Providing completed repeat prescriptions to patient GDPR compliance to ensure Patient confidentiality Book in, amend and cancel patient appointments on the clinical system (system one) Signpost patients to the appropriate service. Answer incoming telephone calls efficiently, ensuring details are accurately recorded and directed as necessary. Cover all reception duties as necessary Process appointment requests from Doctors, Nurse Practitioners, Nurses and Health Care Professionals Deal with visit requests Excellent IT skills Process repeat prescription request in accordance with Practice guidelines Use of clinical system (system one) Confidentiality: In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters.

They may also have access to information relating to the Practice as a business organisation.
