Description

An exciting opportunity for a GP with a specialist interest to work 2 sessions per week in the Local Gender Service. The Local Transgender Service (LTS) is responsible for the onward medication management of adults referred from the All Wales Transgender Service (AWTS) to Cardiff and the Vale UHB.
The post holder will, in conjunction with other team members, be responsible for the initiation and monitoring of medication regimes as directed by specialists within the AWTS, manging their seamless transition into GMS once stabilised.
To provide consultation and appropriate clinical management of patients attending the Local Gender Service. The role will include initiating and optimising hormone treatments in order for patients to be discharged back to their registered GP.
There may also be a requirement to attend relevant team meetings and provide training and education to other members of staff / students and visitors.
To participate in operational and strategic planning for the ongoing development of the service where requested by the Locality Manager/Clinical Board Director
To develop and encourage positive working relationships with local GP practices, housing, social services, voluntary services, drug and alcohol services, mental health, dental, secondary care and all other appropriate departments

  • To provide consultation and appropriate clinical management of patients attending the LTS
  • To initiate and optimise hormone treatments for patients discharged from the AWGS before their prescribing is transferred to their registered GP
  • Prescribing in line with C& V UHB Medicines Management guidelines and protocols to ensure safe prescribing and prudent use of resources
  • The ensure the provision of medication reviews as appropriate and monitoring of repeat prescribing
  • To establish the patient’s full history, including completion of mental health assessment
  • After diagnosis, discuss with patients if they have a particular preference with regards to their transition journey
  • To ensure patients are provided with advice and information regrading routine general and sexual health screening
  • To ensure full physical examination has been offered by either the AWGS or LGS
  • To liaise with Speech and Language and Psychology Services to deliver holistic care for patients
  • To ensure that when a referral is completed, consideration is given to whether there are any co-existing conditions, mental or physical health issues which need to be taken into account
  • To provide telephone consultation and appropriate management of patients who contact the service between appointments
  • To maintain accurate, legible and contemporaneous notes of all patient contacts and contribute to electronic recording and audit as directed
  • To ensure seamless handover of care through electronic/written format to patient’s registered GP
  • To ensure that all medical and other equipment is maintained in good and substantial repair and condition for effective use
  • To work in line with the Values and Behaviours, policies and procedures of the UHB
  • To engage in regular appraisals with Locality Manager/Clinical Board Director
