EDUCATION/EXPERIENCE:

  • Bachelor’s degree in social work, mental health, psychology, or related field required Master’s Degree in social service/Mental Health Counseling preferred
  • Knowledge of community resources
  • Ability to work a flexible schedule including weekends and be on-call as needed
  • Bilingual (English/Spanish or Creole/English) preferred
  • Knowledge, awareness and demonstrate an understanding of the needs of individuals and families that have been impacted by homelessness, poverty, trauma, crisis, violence, medical and other special populations.

Ability to empower individuals to advocate on their own behalf

  • Honorably discharged Veterans or immediate family member preferred

Licenses/Certifications: Valid Driver’s license & proof of insurance

Responsibilities
  • Provide clinical care and coordination for up to 20 Veterans participating in the GPD Program utilizing evidence based practices in service delivery such as intensive case management, Motivational Interviewing,

Harm Reduction, Trauma Informed Care, Critical Time Intervention and Housing First Practices

  • Conduct intake and assessment of new Veterans, face-to-face clinical assessments (i.e. brief behavioral health status examinations, in-depth assessments, and bio-psychosocial evaluations).
  • Provides strength based case management services in the areas of, but not limited to: housing stabilization, money management, community integration, benefits establishment, linkage to community providers for substance use, primary and mental health care, and all other services needed to assist client in reaching their housing stability goals

Program Supervisor

  • Work with Employment and Training department to assist Veterans in accessing employment and training opportunities
  • Develops treatment plans and/or Self Sufficiency Plans and monitors progress towards goals.
  • Completes documentation in the Homeless Management Information System (HMIS) and completes all documentation and paperwork in a timely manner
  • Provides linkage and referrals to community services, agencies, and activities to address resident needs
  • Participates in residential community building activities and coordinate field trips
  • Intervenes and de-escalate residents in crisis and emergency situations
  • Represents organization at Veteran specific meetings, conferences and workshops including: o Annual Veteran Stand Down o Annual CHALENG Meeting o Annual VA Mental Health Summit o GPD BNL Monthly Staffing
  • Conducts monthly meetings with team to address any programmatic, systemic issues that may arise
  • Coordinate and work alongside GPD Therapist and GPD liaison o Receive and process GPD intakes from the Homeless Health Care for Veterans program o Participate in discharge planning and housng search o Network and build working relationship with VA providers and other VA related organizations o Make necessary referrals for GPD veterans (dental, medical, VA benefits, Housing)
  • Collaborate with SSVF programs in transitioning the GPD veterans into permanent housing
