Grants & Finance Coordinator Location: Denver Colorado Pay rate: $31.03 - $52.83 Shift: Monday through Friday Hours: Days 1.0 FTE Status: Exempt At Uptown Community Health Center we improve the health of our community and provide care that respects the dignity of each person, especially those that are underserved. We teach tomorrow's physicians and healthcare professionals to provide excellent medical care with compassion and kindness. Summary: The Grants and Finance Coordinator manages multiple, complex projects related to Quality, Practice Transformation, and value-based reimbursement. This position will closely work with interdisciplinary teams involved in all stages of project development. The Grants and Finance Coordinator will be accountable for the detailed planning of tasks and resources needed to meet project objectives. The Grants and Finance Coordinator also monitors and prepares reports to ensure work is progressing to meet the deliverables on time and on budget. Functions: Upholds the Mission, Vision, and Values of Uptown Community Health Center. Maintains a safe, secure, and healthy environment by adhering to organization standards, policies, and procedures, and to legal regulations; alerting others with potential concerns. Communicates in a professional, clear, effective, and compassionate way with patients, visitors, and coworkers. Both in person, on the telephone, and with written communication. Ensures that the project deliverables are on time, within budget, and at the required level of quality. Responsible for all Charity Care specialty payments and Primary Care Fund Audits. Contributes to the development of cost estimates and prepares for projects and maintains responsibility for managing project budgets. Completes the necessary Breast and Cervical Cancer Program (BCCP) paperwork in a timely fashion according to Women's Wellness Connection standards. Communicates relevant project information with leadership. Performs reporting on Quality Measures to appropriate program sponsors. Completes E-Cast/ e-CRC enrollment for community clinics' patients. Compiles research and updates all Colorado Colorectal Screening Program, Women's Wellness Connection reports from e-Cast/e-CRC to ensure proper patient follow-up after initial visit for community clinics. Assists in keeping associates up to date on Colorado Colorectal Screening Program, Women's Wellness Connection meeting core indicators, and screening and treatment policies. Attends annual Colorado Colorectal Screening Program, Women's Wellness Connection screening/treatment training, monthly conference calls with Colorado Department of Public Health and Environment (CDPHE), quarterly E-Cast training, and quarterly calls with CDPHE. Monitors Webinars and Learning Collaboratives for various programs, and attends as appropriate. Performs additional grant requirements for new grants. Proper and accurate documentation within the electronic medical record: recording patient examination, treatment, and test results. Maintains patient confidentiality and protects medical office operations by keeping information confidential; cautioning others regarding potential breaches. Maintains a clean work area: Clean and stock the rooms in accordance with Kanban. Available to work in person as scheduled and report to work on time regularly and reliably Other duties as assigned Qualifications and Experience Required : High school diploma or GED equivalent One (3) year of experience in healthcare Preferred: Bachelor's Degree OR equivalent experience in Business Administration, OR Data Analysis OR Healthcare Administration, or other related field Two (2) years of experience in healthcare Experience with Epic application and health record Experience working with and sensitivity toward special populations, including those with limited income, limited education, and non-English speaking individuals
