POSITION OVERVIEW

GLC Wellness Center seeks a compassionate, community-oriented Group Personal Care Assistant (PCA) focused on outreach and group facilitation for older adults. Unlike typical PCA roles, this position emphasizes leading engaging group activities, building community connections, and working collaboratively under the supervision of the on-site PMHNP, aligning with her treatment plans.

EDUCATION & CERTIFICATION:

  • High school diploma or equivalent required; associate’s or higher in human services, social work, gerontology preferred.
  • Valid certification in one of the following is strongly preferred:
  • Illinois DSP (Direct Support Person)
  • PCA (Personal Care Assistant)
  • CNA (Certified Nursing Assistant)
  • CPR and First Aid certification (must be obtained within 30 days of hire if not held already).

EXPERIENCE & SKILLS:

  • Minimum 1 year working with seniors, particularly in group settings or outreach.
  • Background in activity planning, facilitation, or community engagement strongly favored.
  • Proven ability to follow clinical instruction under supervision.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to assist with mobility and engage in light physical activity as needed.
Responsibilities
  • Community Outreach
  • Cultivate relationships with senior living facilities, community centers, and local agencies serving older adults; promote GLC programs and services.
  • Group Facilitation (PMHNP Supervised)
  • Design and lead wellness workshops, social groups, educational sessions, and support groups per the clinical goals set by the PMHNP.
  • Adjust activities to accommodate participants’ physical and cognitive needs as detailed in treatment plans.
  • Client Support
  • Offer light personal assistance during sessions (e.g., mobility support).
  • Guide seniors to relevant community resources and transportation.
  • Observe and report clinical or behavioral changes to the PMHNP promptly.
  • Documentation & Compliance
  • Record attendance, session notes, and observation logs.
  • Collaborate on outreach reporting.
  • Adhere to HIPAA, Illinois regulations, and organizational policies.
