Description

Uniting HACC team are looking for a Team Leader for 12 month Maternity Leave Cover role. This will be Full Time role based in Glover Street in Lilyfield. No travel is required for this role.
About This Role
The Team Leader is responsible for leading, supervising, and supporting a multidisciplinary team of Support Workers and Clinicians within Home and Community Care. This role ensures the effective coordination of services, continuous performance development, and delivery of person-centred care. The Team Leader is a key contact point for frontline staff, acts on escalations, and collaborates with other functions including Workforce Management, Care Excellence, and Sales to ensure seamless service delivery. The position involves operational oversight, compliance monitoring, team capability building, and contributing to a positive, inclusive workplace culture.

Key Responsibilities

  • Supervise and support a multidisciplinary team, ensuring performance goals and service standards are met.
  • Coordinate day-to-day service delivery, including managing worker absences, client cancellations, and roster changes.
  • Collaborate with Workforce Management and Sales teams to support efficient rostering and smooth client onboarding.
  • Conduct 1:1 supervision meetings, facilitate team meetings, and oversee training compliance and competency assessments.
  • Provide leadership in WH&S practices, ensuring a safe working environment and incident response as required.
  • Work with the Care Excellence Lead to build clinical capability, identify gaps, and implement coaching or supervision plans.
  • Monitor and interpret service data and audits to drive service improvements and compliance actions.
  • Act as escalation point for team and client concerns, resolving conflicts, managing feedback, and promoting client-centred care.

Qualifications & Experience

Essential

  • Qualifications in Aged Care, Community Services, Nursing, or a related health field.
  • Experience in leading teams in a home care or community services environment.
  • Strong understanding of person-centred service delivery, WH&S, and rostering practices.
  • Demonstrated ability to coach, support, and supervise staff.
  • Excellent communication and conflict resolution skills.
  • Proficiency in using rostering or client management systems.
  • Knowledge of aged care standards, relevant legislation, and continuous improvement frameworks.

Desirable

  • Formal leadership or management training (e.g. Certificate IV or Diploma in Leadership & Management).
  • Experience working in integrated teams across clinical and non-clinical roles.
