**Job Title:** Haitian Creole Care Coordination Specialist **Location:** Remote (Home-based in Haiti only) **Employment Type:** Full-Time (40 hours/week) **Work Schedule:** 9:00 AM to 5:00 PM EST (flexible if needed) **Position Overview:** We are seeking an individual freelancer (not an agency or BPO) based in Haiti to join our team as a Care Coordination Specialist. The specialist will support Haitian Creole-speaking members and caregivers through outbound calls, intake follow-ups, and new referral onboarding. This role is essential in reinforcing program expectations, confirming daily visits, addressing no-shows, and performing compliance follow-ups for at-risk members.
