SKILLS/KNOWLEDGE/EXPERIENCE

  • A college degree in social work or related field is required
  • 2-3 years of case management experience working within a harm reduction program, substance use counseling, housing/homelessness, behavioral health, or other relevant settings. Peer Recovery Specialist training or certification is a plus
  • An applicant with lived experience will be given consideration however and must have a minimum of two years of recovery from an addiction or co-occurring disorder
  • Ability to track and monitor the effectiveness of programming and enter into different databases
  • A valid driver’s license
  • Excellent communication skills: written, verbal, and listening
  • Strong time management and organizational skills
  • Ability to work independently and on a team
  • Demonstrated knowledge of Microsoft Office programs and the ability to use and administer various databases
  • Ability to handle diverse person-centered support services
  • Experience working with community members in a supportive, educational role
  • Ability to follow through on assigned tasks
  • Experience in planning or facilitating events and activities
  • Must be punctual, reliable, self-motivated and possess initiative to improve current programming
  • Ability to relate, communicate, and respond competently and positively with an understanding of Indigenous traditions, lifestyles, and worldviews with a focus on Anishinaabe culture or willingness to learn Indigenous worldviews and philosophies in program development and implementation

Responsibilities

PRIMARY PURPOSE:

Northwest Indian Community Development Center (Aazhoomon) Ombishkaa Harm Reduction Program Coordinator is part of the Ombishkaa “Rising Up” Reentry Program team and will work collaboratively within our reentry and recovery community pathways program. These pathways will welcome community members back through advocacy, mentoring, and facilitating access to resources related to harm reduction models of care. The Ombishkaa Harm Reduction Community Navigator will work with community members to provide stability and empowerment on their recovery journey. The community navigator will be responsible for developing and presenting community education and outreach on behalf of Northwest Indian Community Development Center.

DUTIES AND RESPONSIBILITIES

  • Connect people to services by helping members gain access to counseling, treatment, and overdose education and services
  • Work one-on-one with individuals to create care plans that set goals to achieve positive family change as defined by the member
  • Distribute and recover harm reduction supplies such as safe injection equipment, opioid overdose reversal medications, and provide education/information and support on safer drug use, safer sex, HIV, hepatitis C, and STIs
  • Conduct outreach and survey substance abuse disorder conditions within the community
  • Represent, participate, and/or facilitate community meetings to provide education, support, and access to community resources
  • Co-facilitate groups on topics like harm reduction, wellness, and healthy decision making
  • Complete training or professional development opportunities as it pertains to the position
  • Reach designated assigned monthly program goals and outcomes and complete required reports as required Communicate and report all pertinent information on assigned monthly program goals and outcomes that are promptly and correctly entered into the database system
  • Keep and maintain accurate and confidential records
  • Any other duties as assigned
