About the Role

As a Care Partner, you will:

• Manage and coordinate a caseload of Support at Home, NDIS, and private clients
• Conduct client assessments, reviews, and care planning
• Build meaningful relationships with clients, families, and healthcare professionals
• Ensure clients receive services that meet their goals and changing needs
• Monitor budgets, services, compliance, and client outcomes
• Advocate for clients and provide exceptional customer service

About You

You’ll shine in this role if you’re organised, empathetic, and passionate about improving lives.

Must-haves:

• Experienced in aged care, community care, case management, or care coordination
• Background in community or aged care service coordination
• Great communication and planning skills
• Ability to work independently and as part of a team
• Current driver’s licence
• Able to obtain a Police Check & NDIS Worker Screening

Bonus points if you have:

• Nursing qualifications (RN or EN)
