Role Summary The HCBS Client Care Coordinator serves as the primary relationship owner for assigned members and providers, helping ensure consistent, high-quality services from intake through ongoing care coordination. Key responsibilities include coordinating intake activities; attending planning meetings and capturing habilitation goals/tasks in real time when possible; documenting communication and service activity; supporting provider orientation and re-orientation; tracking member/provider needs; and following up to support service continuity and retention. Required qualifications/skills: - Direct Support Provider experience (required) - Background in care coordination, case management, intake, customer service, healthcare support, human services, or related work - Compassionate, organized, responsive; strong follow-through and attention to detail - Clear written/verbal communication; comfortable working independently and with a team - Bilingual English/Spanish (preferred) If you’re ready to contribute to accountable, relationship-based coordination, apply today. Daily Workflow Your schedule is Monday–Friday, 8:30–5:00 (with flexibility based on client meetings), with work completed onsite and at community meetings. You’ll begin by checking messages, recent updates, and pending follow-ups so priorities are set for the day. Much of your time centers on planning meetings, where you partner with members, families, providers, and internal staff while keeping information organized for prompt entry. Between meetings, you’ll keep communication on track, confirm next steps, and ensure items affecting service continuity and retention are resolved.
