Job Details Job Location: SECOND CHANCES-HOPEWELL - HOPEWELL, VA 23860 Position Type: Full Time Job Shift: Days Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) Quality Assurance & Clinical Case Manager (Hybrid) Second Chances Comprehensive Services is seeking a highly skilled and licensed LPC or LCSW to serve in a combined Quality Assurance and Clinical Case Manager role. This hybrid position is responsible for delivering direct clinical and case management services while leading quality assurance and compliance efforts across community-based mental health programs. Under the clinical supervision of the Clinical Case Director, the Clinical Case Manager provides counseling services, completes comprehensive biopsychosocial assessments, oversees treatment planning, and ensures services are delivered in accordance with agency policies and VDBHDS/DMAS regulations. In addition, this role provides direct supervision to assigned QMHP staff, conducts documentation reviews, performs quality compliance audits, and recommends system and process improvements to enhance service quality, operational efficiency, and regulatory compliance. Essential Duties and Responsibilities Clinical & Case Management Functions Provide comprehensive case management and therapeutic counseling services to adults, children, and families within community-based mental health programs. Complete timely and accurate biopsychosocial assessments, diagnostic impressions, and ongoing clinical evaluations. Develop, review, and update Individualized Service Plans (ISPs) in collaboration with clients, families, and interdisciplinary teams. Ensure all clinical services are person-centered, trauma-informed, and aligned with best practice standards. Maintain compliance with ethical standards, confidentiality requirements (HIPAA), and professional licensure regulations. Quality Assurance & Compliance Perform routine quality assurance and compliance audits of clinical records and service documentation. Review and sign off on counselor notes, supervision notes, ISPs, assessments, and other required client documentation. Ensure documentation meets established company quality standards and all applicable state and federal regulations, including VDBHDS and DMAS requirements. Identify trends, gaps, and risks in documentation and service delivery; prepare and present quality data, findings, and compliance reports to leadership. Propose corrective actions, performance improvement plans, and preventative strategies to address deficiencies. Staff Supervision & Leadership Provide direct clinical supervision to assigned QMHP staff in accordance with licensure and regulatory standards. Conduct individual and group supervision sessions, offering clinical guidance, feedback, and professional development support. Review, approve, and provide corrective feedback on staff documentation and service delivery. Support staff adherence to policies, procedures, and ethical standards. Process Improvement & Risk Management Recommend and assist in implementing measures and process improvements to enhance clinical operations and service delivery. Promote system reliability and workflow efficiency while minimizing errors, service disruptions, and potential fraudulent activity. Collaborate with leadership and interdisciplinary teams to strengthen internal controls and quality monitoring systems. #INDENH Qualifications Qualifications Required Active and unrestricted LPC or LCSW license in the Commonwealth of Virginia. Master’s degree in Counseling, Social Work, or a closely related mental health field. Minimum of 2–3 years of clinical experience in community-based mental health services. Demonstrated knowledge of VDBHDS/DMAS regulations, Medicaid documentation standards, and clinical compliance requirements. Prior experience providing clinical supervision to QMHPs or other behavioral health staff. Strong auditing, analytical, and documentation review skills. Excellent written and verbal communication skills. Preferred Experience in quality assurance, utilization review, or compliance monitoring. Familiarity with electronic health record (EHR) systems. Experience developing quality improvement plans and performance metrics. Strong organizational skills with the ability to manage multiple priorities independently
