Job Title: Health and Wellbeing Assessor
Location: Croydon
Division: Adult Care Services
Team: Adult Social Care – LIFE Service
Contract: Permanent, Full-Time (36 hours/week)
Salary: Grade 6 – £32,925 to £33,957

We’re looking for a dedicated and knowledgeable Health and Wellbeing Assessor to join our LIFE (Living Independently For Everyone) service. This role is key to helping individuals regain independence and live fulfilling lives at home.

You’ll work closely with a multidisciplinary team including Social Workers, Occupational Therapists, Physiotherapists, and Intermediate Care Nurses supporting them by carrying out delegated assessments and interventions. You’ll be based in the community, hospital, or people’s homes, working independently across a variety of settings.
