Location: Croydon
Division: Adult Care Services
Team: Adult Social Care – LIFE Service
Contract: Permanent, Full-Time (36 hours/week)
Salary: Grade 8 – £37,443 to £39,264

Are you ready to take on a rewarding challenge? We’re seeking a skilled and committed Health and Wellbeing Assessor to join our LIFE (Living Independently For Everyone) service. This role plays a vital part in supporting individuals to regain independence and live confidently at home.
As a trusted assessor, you’ll work alongside Social Workers, Occupational Therapists, Physiotherapists, and Intermediate Care Nurses. You’ll carry out delegated tasks and assessments, and at this level, you’ll also be responsible for supervising Grade 6 staff.

You’ll work autonomously across a range of settings, including hospitals, community locations, and people’s homes delivering person-centred support that empowers individuals to reach their full potential.
