As a Health and Wellness Coordinator at Castle Hill, you’ll play a key role in supporting older residents to age in place with dignity, choice, and connection. You’ll provide personalised guidance, link residents with care and wellbeing services, and foster a strong sense of belonging within the community.
Your opportunity
In this full-time role, you’ll be a cornerstone in understanding and addressing the care needs of our residents while promoting health, wellbeing, and social connection.

With care and professionalism, you will:

  • Undertake health and wellbeing assessments for new and existing residents
  • Build rapport and identify services that best meet residents’ needs
  • Support residents through the My Aged Care process, including referrals and advocacy
  • Work closely with the Onsite Care team to meet health and social needs
  • Organise a calendar of wellbeing programs, groups and events that strengthen social connection

What you’ll bring
You’re compassionate, proactive, and motivated by a genuine desire to support older Australians to live well and independently.

You will need:

  • Strong interpersonal and communication skills
  • Ability to build trust and advocate for residents’ needs
  • Knowledge of My Aged Care and community aged care services (desirable)
  • Organisational skills to plan and coordinate events and programs
  • A collaborative approach and commitment to person-centred care
