We are currently seeking a Health and Wellness Administrator to support our Central Pennsylvania region. This position plays a key role in coordinating medical and wellness services for the individuals we support across our residential programs. The Health and Wellness Administrator (HWA) provides coordination of medical appointments, medication administration oversight, and individualized wellness planning. The HWA also serves as the HRST Administrator, conducting assessments and collaborating with team members to address identified health risks. This salaried, exempt position is part of the on-call rotation and works closely with nursing, program leadership, and direct support staff to ensure high-quality health and wellness outcomes. Responsibilities include: Coordinate and attend key medical appointments for individuals, including providing transportation if needed Maintain and review medical records and ensure documentation compliance, including medication audits Serve as HRST Administrator: conduct assessments, monitor scores, and train staff Deliver and oversee Medication Administration training and compliance Support the development of individual wellness goals and WRAP plans Provide education to individuals and DSPs on wellness-related goals Collaborate with teams to address med errors, care planning, and risk mitigation Track and maintain records through Therap and other documentation systems Participate in the on-call rotation and provide after-hours support as needed Qualifications Qualifications: Associate’s or Bachelor’s degree in Human Services, Health Admin, Nursing, or related field preferred Minimum 2 years’ experience supporting individuals with IDD, or working with other special populations Experience with 6400 regulations, Therap, HRST, and medical coordination strongly preferred Must be certified as Medication Administration Trainer within 90 days Must be at least 21 years of age with valid driver’s license and 3 years’ driving experience and reliable transportation Ability to travel between and within Dauphin and Lebanon Counties, and occasional travel to Lycoming County required Join our team and make a meaningful impact in the lives of others.
