JOB SUMMARY

We are seeking a compassionate and dedicated Health Care Assistant to join our team. The ideal candidate will provide essential assistance to individuals in need of care, ensuring their comfort, safety, and well-being. This role is pivotal in enhancing the quality of life for those we support, whether in a residential setting, nursing home, or through home care services.

SERVICE USER DESCRIPTION

Service user 39 years old, living with her mother, she has been diagnosed with Multiple Sclerosis, Meningitis, asthma, pneumonia, and sepsis. She has a healing grade 3 pressure ulcer on her sacrum towards left cheek. She is on PEG feeding to meet her nutrition and hydration needs, and on foley’s catheter to handle her continence. She is severe cognitive impact and may become tearful in the night. She has frequent seizure episode and on regular anti epileptics, if the seizure prolongs, she will be administered with Buccal Midazolam as an Emergency drug of choice. Her pressure ulcers are managed with frequent repositioning and on pressure relieving air mattress. She is Non-verbal and unable to indicate care needs, yet she can nod at times but not always accurate. Due to Multiple Sclerosis and Frontal Lobe Brain damage. She is severely cognitively impaired. Recognises family and regular carers due to facial expressions and body language.

Responsibilities
  • Provide patient care and support to individuals in various settings, including senior care and assisted living facilities.
  • Assist with daily living activities such as bathing, dressing, and meal preparation.
  • Administer medication as prescribed and monitor health conditions.
  • Engage in behaviour management techniques to support individuals with challenging behaviours.
  • Perform heavy lifting when necessary to assist clients with mobility or transfers.
  • Maintain a clean and safe environment for clients by adhering to health and safety regulations.
  • Collaborate with healthcare professionals to ensure comprehensive care plans are followed.
  • Document care provided and report any changes in client condition to the appropriate personnel.
