Description

To support the practice nursing team and the practice care coordinator in the delivery of nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The health & community care assistant will deliver and assist clinical staff in the provision of treatment, preventative care, health promotion and patient education both in a clinical environment and to housebound patients.
The following are the core responsibilities of the Health & Community Care Assistant. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
· Support the practice nursing team with health promotion programmes Undertake new patient health checks
· Undertake NHS Health checks as required
· Carry out baseline observations such as pulse oximitery, blood pressure, temperature, pulse rate, recording findings accurately
· 24 hour Ambulatory blood pressure monitoring and downloading of data
· Carry out BMI checks as directed
· Undertake ECG’s, download data for clinical interpretation
· Act as a chaperone as required
· Phlebotomy – Clinical and Housebound patient population
· Ensure specimens are tested where necessary, recorded and ready for onward transportation
· Undertake D-Dimer testing as required as well as quarterly NEQAS quality checks
· Ensure all clinical rooms are adequately stocked and prepared for each session
· Ensure fridges are cleaned routinely in accordance with extant guidance
· Ensure fridge checks are undertaken in accordance with the duty rota
· Ensure clinical waste is removed from clinical areas and sharps bins replaced in accordance with the practice IPC policy
· Deliver opportunistic health promotion where appropriate
· Clinic and housebound immunisations: Flu, Shingles, Pneumocooal & Covid
· Conduct B12 injections in clinical setting
· Conduct dressings and both suture & clip removal in clinical setting
· Work flexibly both in clinical and housebound situations
· AIR Team referrals – keep a record of all referrals & make contact with all patients
· EMAS safeguarding referrals – determine who needs to follow up.
· Post-discharge housebound appointments to establish care plans and escalation.
· Attend local groups – to offer proactive signposting and clinical escalation.
· Support with Wellbeing Walks – 3rd Weds of each month.
· Responsible for maintaining local groups and events timetable.
· Attend Neighbourhood Team meetings.
· Develop knowledge of local services and understand how services are accessed.
· Maintaining the Dementia Register.
· Understanding the degree of frailty, mild, moderate or severe and the 6 frailty syndromes to follow the correct proportionate response to patients needs.
· Promote safety and wellbeing and where people are experiencing or at risk of harm apply Lincolnshire’s multi agency safeguarding adults procedure, understanding what is important to the person to protect them from harm.
Support patients with problematic polypharmacy, reduce hospital re-admissions, waste and demand on GPs. Identify, prioritise, coordinate, track, and record medicines optimisation opportunities.

Pay: £13.00-£13.50 per hour
