Responsibilities

WHAT DO I NEED TO BRING TO THE ROLE?

  • Relevant registration: You must be a Registered or Enrolled Nurse with the Nursing and Midwifery Board of Australia (NMBA) via AHPRA, or a qualified Aboriginal Health Worker.
  • Strong clinical skills: Demonstrated ability to conduct comprehensive assessments, develop care coordination plans, deliver safe and responsive practice, and evaluate outcomes effectively.
  • Recovery-oriented mindset: A solid understanding of recovery-based approaches for individuals with severe and persistent mental health challenges, and the ability to work collaboratively with individuals and their families to support informed decision-making and active participation in care.
  • Knowledge of chronic care: Proven competencies and/or understanding of the complexities involved in supporting individuals with chronic health conditions.

What will you be doing?

  • Coordinating care for individuals with complex mental health and chronic physical health needs — including initial assessments, goal setting, progress tracking, and planned program exits.
  • Using a person-centred approach to support participants, families, and carers in developing and achieving recovery-focused goals.
  • Delivering health and wellness education to consumers and the broader community, with a focus on physical health topics.
  • Facilitating community sessions and anti-stigma initiatives to raise awareness among the public and health professionals about the vital connection between physical and mental health for overall wellbeing
