JOB OVERVIEW

The School of Opulence is on a mission to restore the holistic health of Queer People of Color (QPOC) by advocating for vital resources and delivering transformative wellness programs. We are currently seeking a dedicated Healthcare Navigation Specialist with a focus on TGD/Gender Diverse care to join our team. In this role, you will serve as a key support system for individuals navigating the complexities of healthcare and holistic wellness. Your work will directly support access to life-saving medical services while addressing the broader social factors that impact our community’s health outcomes.

REQUIRED EXPERIENCE & QUALIFICATIONS

  • Deep knowledge of HIV treatment and related healthcare protocols
  • Strong communication and relationship-building skills with diverse communities
  • Experience with case management, patient advocacy, or healthcare navigation
  • Skilled at navigating insurance systems and community health resources
  • Bilingual or multilingual skills are a strong asset
  • Familiarity with issues impacting QPOC and TGD communities preferred
    Job Type: Part-time
    Pay: $23.00 - $24.00 per hour
    Expected hours: 25 per week

Responsibilities
  • Patient Outreach & Engagement
  • Proactively engage individuals at risk for HIV through compassionate, affirming outreach
  • Promote testing, care linkage, and long-term treatment adherence
  • Care Coordination
  • Schedule appointments and manage referrals for HIV-related care
  • Coordinate follow-ups and collaborate with healthcare providers to ensure continuity

  • Support patients in understanding and enrolling in appropriate insurance plans
  • Assist with HIV medication benefit management
  • Education & Counseling
  • Provide comprehensive education on HIV, treatment plans, medication adherence, and lifestyle wellness
  • Empower patients to make informed health decisions
  • Social Needs & Support Services
  • Assess social determinants of health (housing, food security, etc.)
  • Connect patients to appropriate community resources
  • Advocacy & Case Management
  • Represent patient interests within healthcare systems
  • Navigate complex care scenarios with a trauma-informed approach
  • Data & Reporting
  • Maintain accurate documentation
  • Track patient outcomes to improve program effectiveness
