The incumbent will function as a member of an interdisciplinary surveillance team investigating complaints from resident/facility/family representative for multiple downstate counties, conducting interviews of residents, family representatives and the Inter disciplinary team; reviewing medical records, care plans, discharge planning and quality assurance; and conducting post survey revisits both on site and off site. The incumbent will be involved in immediate jeopardy situations as they arise and will also be part of a healthcare team for monitoring health emergencies; participate on recertification surveys as needed. The incumbent will be conducting onsite post survey revisits; responsible for writing and reviewing statements of deficiencies and case summary reports; reviewing data received from Central Intake on the ASPEN system, entering surveillance data into ASPEN, and uploading documents and summaries into electronic filing systems; reviewing data received from Central Intake on the ASPEN system, entering surveillance data into ASPEN, and uploading documents and summaries into electronic filing systems.
