Job Overview
Momba Home Care is seeking a dedicated, professional, and passionate Health Coach to join our team. As a Health Coach, you will work closely with individuals with disabilities to provide guidance and support in achieving their day-to-day activities, utilizing your expertise in motivational health coaching, and health, you will develop personalized care plans tailored to each client’s needs.

Duties:

• Collaborate with clients, caregivers, and the clinical team to create achievable care plans for sustainable health improvements
• Monitor client progress and adjust client specific care plans and caregiver tasks as needed
• Create and maintain positive relationships with case managers.
• Home visits to clients semi-annually and as needed.
• Markets company’s services and increases community awareness
• Timely data entry and upload of new packets
• Report progress daily to supervisor
• Attend daily meetings and present case load information
• Timely incident reporting
• Maintain a high level of integrity, adhering to ethical standards and HIPPA confidentiality guidelines.

Qualifications:

• Experience working in/around home health industry.
• Familiarity with Medicaid and/or waiver programs such as EDWP, CCSP, COMP, NOW, VA, etc.
• A level of understanding of the care and needs of elderly and physically/developmentally disabled persons.
• Excellent communication skills and ability to build rapport with clients.
• Willingness and ability to use mobile applications.

Pay: $50,000.00 - $55,000.00 per year
