As a Health Coach, you will manage a set amount of patient population by reviewing trends of patient vital readings and communicating with a cadenced patient outreach process. Positively impacting patients’ lives by improving patient outcomes, reducing hospitalizations, and avoiding unnecessary trips to the doctor’s office. You are seen as an extension of the Provider’s office and as such, client delight is an integral part of your work.

MINIMUM QUALIFICATIONS:

  • Medical Assistant Certification (Required)
  • A minimum of two (2) years of experience in a clinical setting
  • Experience with Microsoft 365, Teams, and Virtual Meeting Platforms
  • Ability to self-motivate and work independently with minimal direction
  • Exceptional critical thinking skills
  • Extraordinary interpersonal communication abilities
  • Must act with honesty and integrity
  • Motivational Interviewing/Health Behavior Change experience a plus

PHYSICAL REQUIREMENTS:

The physical activities of this position may include sitting for long periods of time, hearing, walking, reaching, grasping, and repetitive motion. The Health Coach is required to have close visual acuity including color, depth perception, and field of vision to perform an activity, such as assessing computer data, analyzing data and figures; viewing a computer screen; or extensive reading

Responsibilities

Conducting initial consultations and individualized patient care
Evaluate vital readings and escalate trends of concern
Develop Care Plans tailored to each patient needs with actionable goals
Provide Patient Outreach on a weekly cadence to help patients meet their health goals
Develop strong relationships with patients and Health Care Providers
Educate patients and meet patient engagement program goals
Communicates orally and in writing with patients, physician and staff as indicated
Prioritizes and organizes daily workflow other assignments/projects to meet commitments and deadlines
Willingness to work with our Product Development team so our platform can meet the needs of our patients
and help our business be successful
Reimbursable activities with the patients include use of their device and time spent with the patient
Provide daily and weekly Leaderboard results
Performs other duties as required
