POSITION SUMMARY

Moda Health Coaches develop and implement behavioral-based coaching programs and health care navigation support, including condition-specific disease management (health coaching), for Moda members. This position collaborates with internal and external healthcare services teams, vendor point solutions, and community-based organizations to ensure continuity and coordination of care.

REQUIRED SKILLS, EXPERIENCE & EDUCATION:

  • Bachelor’s degree, preferably in healthcare-related field.
  • Ability to work collaboratively and independently.
  • Excellent communication, interpersonal, and presentation skills.
  • Strong cultural competence and understanding of the importance of using Diversity, Equity, and Inclusion (DEI) concepts to guide and inform work.
  • Thorough knowledge and understanding of health behavior change theory.
  • Experience with Microsoft Office products.
  • Ability to learn proprietary databases and maintain confidentiality.

PREFERRED SKILLS, EXPERIENCE & EDUCATION:

  • Master’s degree in a related field.
  • Minimum of one year of experience in field related to health coaching.
  • Specialty credential such as NBC-HWC, RDN, LCSW, or LPC.
  • Experience in health insurance or similar industry.

Primary Functions:

  • Works one-on-one with members managing, or at-risk for developing chronic health conditions by providing health coaching via multiple communication channels, including phone, email, and mail to support disease prevention and management.
  • Using evidence-based guidelines, provides education and support to help members set and achieve health-related SMART goals and increase their capacity as a self-manager of their health.
  • Assesses member health through initial consultations and evaluations; including exploring visions, motivation, readiness to change, and health history. Routinely documents member progress, changes in health behaviors, and/or gaps closures in EHR and customized database systems.
  • Partners with internal and external healthcare services teams to achieve triple-aim goals of improving healthcare experience, outcomes, and reducing avoidable cost.
  • Contributes to the selection, development, and review of health education materials, and other group and member-facing wellness communications.
  • Occasionally provides health education presentations to group employees on topics related to healthy lifestyle and condition management.
  • Maintains knowledge of vendor health management programs and refers members to programs, as needed.
  • Performs other job-related duties, as assigned
