Description Document medical encounters for appointments to appropriate software. Ensure follow up appointments are made and communicated with appropriate parties when needed. Upload completed medical encounters. Audit for medication changes as needed. Communicate with pharmacy when medication changes are made. Check medications on PHS to ensure changes have been documented correctly and that staff are administering meds according to policy. Communicate with appropriate staff any changes or upcoming appointments. Print monthly MARs. Keep each individual’s medical information up to date — Plan of Care, prescriptions, insurance, annual appointments. Respond timely to requests for medical information from agency staff, ISC, MDs, and Conservators. Ensure TennCare renewals are completed accurately and timely. Obtain psychotropic medication consents as needed. Ensure Human Rights and Human Rights Restrictions are being followed Train/Retrain staff in the homes on medication changes and medication variances to ensure the safety of the people supported. On call as scheduled. Requirements High school diploma or equivalent Requires basic language, written and computer skills necessary for communication and documentation
