Description The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y. Jordan Health is now seeking a Health Educator who, under the guidance of the Practice Manager, is primarily responsible for providing information on healthy lifestyles, STI prevention, HIV Prevention, safe dating practices, domestic violence/sexual assault, sexuality, human trafficking, Contraception, and other issues as they pertain to the age group served, ages 12 to 25 in both the Parsells location and at the SNHC at Franklin. Requirements Requirements The Health Educator will ensure to: · Plan, develop, implement and update health and community outreach programs. · Conduct community outreach in order to build client numbers and promote health education. · Complete the annual CPEP Health Education report and coordinating quarterly information and education committee meetings. · Collaborate with others to accomplish grant goals, such as Comprehensive Pregnancy Prevention Program or similar organizations or groups. · Provide direct counseling services to clients in the area of contraception, HIV, STDs, problem pregnancy, sexuality, sexual assault, substance abuse, and other health related issues by referral from providers. Education And Experience Required: · Bachelor’s degree or prior experience conducting health education and outreach · Facilitation skills · Content-specific knowledge related to family planning and sexual and reproductive health · Public speaking skills · Program evaluation knowledge · Counseling skills including motivational interviewing skills · Ability to conduct a basic needs assessment · Understanding of program theory · Critical thinking skills Licenses And Certifications: · None required but CASAC certification preferred. · BLS through the American Heart Association · NYS Infection Control certificate Special Skills, Knowledge Required: · Must possess a broad background in health education with assessment and counseling skills pertinent to adolescent needs and developmental tasks. · Must have the ability to work with adolescents individually and in group and family sessions. · Should have a broad background in family planning methodology and the ability to provide effective and accurate presentations to adolescents and the community at large. · Must possess the ability to initiate and maintain effective community linkages. · An in-depth understanding of HIV and STI information is required.
