POSITION OVERVIEW:

We are seeking a motivated and compassionate Health Educator / Health Coach to join our collaborative healthcare team. This position involves working directly with patients to develop and implement personalized meal plans, exercise routines, supplement recommendations, and behavioral strategies that promote weight loss, disease management, and overall well-being.
The ideal candidate will bring both clinical insight and a motivational approach to patient care, working alongside our providers to support long-term health outcomes.

QUALIFICATIONS

  • 1–2 years of experience working in chronic disease prevention or health coaching, preferably in a clinical or wellness setting.
  • Strong understanding of diabetes and lifestyle medicine approaches.
  • Experience with motivational interviewing or similar patient engagement tools.
  • Excellent interpersonal and communication skills.
  • Ability to work both independently and as part of a collaborative care team.
