HEALTH EDUCATION

• Develop tailored one-on-one health coaching to individuals around their stated health goals primarily via phone, email, video, and in person.
• Partner with members to co-create action plans and SMART goals to meet their health needs, monitoring and providing accountability to progress in achieving their goals.
• Maintain accurate and up-to-date documentation of all coaching encounters.
• Conduct regular outreach to drive patient engagement, including follow-up calls to discuss health, assessment results, and participation in biometric screening events, employee health fairs, lunch and learns, and monthly webinars as needed.
