We welcome individuals of all ages, from infants, school-age children (5+), youth and adults. The Floating Hospital is a Federally Qualified Health Center and the largest provider to homeless families living in family shelters and domestic violence safe houses throughout all five boroughs. The Position: Under the supervision of the Program Director, the Health Educator will be a member of The Floating Hospital’s Medical Unit at the PATH Center in the Bronx, located at 151 East 151st Street. The ideal candidate will educate and deliver relevant and pertinent health education resources to all families receiving services from the Dept. of Homeless Services’ Medical unit. Main Responsibilities: Conduct health education counseling for adolescents and adults in various settings. Complete linking and outreach during each health education session with families. Secure medical appointments at the main site if needed. Complete health education monthly reports and relevant weekly reports to submit to the referral sources and program director. Complete and distribute referrals daily. Maintain Nurse-Family Partnership logs and follow up with client placement. Maintain the Newborn Visiting Program log. Maintain and replenish an inventory of health education supplies. Requirements Minimum bachelor’s degree in health education, public health, nursing, or a related field. A Master’s degree is desirable. Minimum 3 years in the healthcare area, providing health education sessions to adults and children. Substantive experience teaching people about behaviors that promote wellness. Develop strategies to improve the well-being of individuals and communities. Fluency in written and spoken English & Spanish is preferable. Possess group facilitation and group dynamic skills. Possess excellent written, and oral communication. Strong public speaking skills.
Responsibilities
