Description:
DESCRIPTION
The Health Information Coordinator/Appointment Coordinator will oversee all client-related health/medical information and conduct and facilitate health and medical functions including appointments. The individual in this role is responsible for scheduling and completing medical appointments, ensuring compliance related to client appointment documentation, medication receipt, inventory, and dispensing, and updating the Health Risk Screening Tool for clients.
REQUIRED KNOWLEDGE AND DEMONSTRATION OF:

Advanced Microsoft Office skills including Word, Excel, Access, Publisher and Power Point

Responsibilities:
Oversee day-to-day client health and medical related follow-up for emergency room, urgent care visits, annual screening and medical appointments.
Conducts and coordinates client medical visits and ensures appropriate documentation is obtained and completed, ensures appropriate orders are obtained from providers, and communicates to key stakeholders, including guardians, leadership, employees who work with clients, etc.
Ensures overall client health, safety and welfare.
Ensures follow-through on client discharge orders and provider instructions including but not limited to follow-up appointments, equipment orders, medication orders, etc.
Assists with new client onboarding and transition, including but not limited to assistance with gathering appropriate order and medical information, scheduling medical appointments within seven days of admission to the program.
Maintains client medical files and compliance and enters information into appropriate databases and share point files (Teams).
Ensures that all medical visit and discharge forms and documentation for all clients are complete, accurate and received in a timely manner.
Communicates with and provides information to clients, case managers, guardians and other key stakeholders on a consistent basis and following appointments.
Coordinates and conducts medical appointments and medical care for clients.
Communicates with team members regarding changes in the client’s medical or mental condition and needs and coordinates psychiatric information with the psychiatric rehabilitation practitioners.
Writes progress notes, summaries, and coordinates client discharge information.
Communicates with and develops relationships with providers to ensure continuity of care and service.
Reports significant/major incidents or accidents and notifies proper authorities of circumstances or situations that present possible health hazards.
Ensures medication is ordered, received, inventoried and taken to the homes for administration in a timely manner.
Completes incident reports related to client medical information.
Ensure client Health Risk Screening Tool (HRST) is consistently updated with client medications, medical, behavioral and other health, safety and welfare information.
Understands and has the ability to execute state and federal regulations that relate to client services, client health safety and welfare, employee training and human resource compliance.
Maintains forms, documents, files, etc.
Maintains files and documents in an organized format.
Completes clerical and administrative duties as necessary.
Other duties as assigned.
