Job Details Job Location: Community Health Improvement Center - Decatur, IL 62521 Position Type: Full Time Education Level: High School Salary Range: $15.00 - $20.00 Hourly Job Category: Admin - Clerical Job Summary: Independently performs specialized or advanced health information activities necessary to organize, maintain, and use electronic patient health records. Activities required include research, analysis, and compilation of findings into summarized reports that will be shared within and external to the Health Information Management Department. Positions at this level have high customer service, strong analytic and problem-solving skills, and require interpretation and explanation of policy and external requirements related to, privacy, release of information, positive patient identity, and other department and organization-wide functions. Specific job responsibilities are based on the service unit within Health Information Management assigned. Expert computer skills to navigate and query multiple electronic record systems. Supervisory Responsibilities: None Qualifications Duties/Responsibilities: Ensure all documents are scanned into the patient’s chart with accuracy Validate and fulfill requests and authorizations for the release of medical information to individuals or agencies according to regulations in a timely manner. Retrieves and releases patient information to patients, families, providers and other requestors in accordance with HIPAA; State and Federal regulations and laws; and organizational policy. Utilizes a variety of means to deliver patient information including mail, fax, email, patient portal and in-person Places dictation, lab reports and other documentation in charts Retrieve patient medical records from outside facilities with appropriate releases Sorts through mail and faxes and distributes to appropriate staff Retrieves, sorts and prepares medical record documents for scanning Uploads clinical documents to the EHR or document imaging software which are received in a variety of electronic methods Maintains confidentiality of patient information and documents in accordance with federal and state regulations Performs other duties as required Competencies Ability to follow established procedures and maintain high level of organization Ability to learn reasonably quickly Ability to maintain confidentiality Ability to multi-task Ability to relate to persons of various economic levels and cultures Ability to remain relaxed and calm under stressful situations Ability to speak and write fluently in English; Spanish desirable Ability to work independently and cooperatively Above average level of interest in working with people and data Cooperative and non-competitive attitude with ability to take initiative Demonstrates integrity, professionalism and superior customer service skills while working in a fast-paced, moderately stressful environment Excellent alphabetizing skills Patient and staff friendly with excellent communication skills Proficiency in use of telephone, computer, and basic office equipment Education and Experience: High School Diploma or equivalent Degree in Health Information Technology preferred Two (2) years of successful experience in a clinical setting preferred Physical Requirements: Prolonged periods sitting of at a desk and working on a computer. Must be able to lift up to 15 pounds at times Fine motor coordination sufficient to perform manipulative skills Moderate physical exertion including frequent walking, standing, bending, reaching and carrying moderately heavy loads (up to 25 lbs.)
