We are seeking a dynamic, highly motivated Health Navigator who demonstrates our values of compassion, accountability, responsiveness, and empowerment, to our clients, families, and each other.
Under the Resettlement Assistance Program (RAP), the Government of Canada assists Government-Assisted Refugees (GARs) upon their arrival in Canada by providing direct financial support and funding immediate and essential services. The Wesley Resettlement Assistance Program, located in Hamilton & Brantford, ON, delivers RAP immediate and critical services to clients generally within four to six weeks of arrival in Canada.
The Health Navigator will support government-assisted refugees in enhancing their well-being and mental health. This role aims to help individuals manage stressors, trauma, and other challenges that may impact their daily functioning. The Health Navigator will also work closely with community partners and provide referrals to appropriate services as needed.
This position is a unionized, full-time, permanent position. The rate of pay is $29.86 per hour. Working hours are 37.5 weekly.

Responsibilities
  • Facilitate opportunities to engage individuals in a supportive manner that will support discussion around stressors, trauma, and other issues that may impede healthy functioning.
  • Provide appropriate referrals to mental health services in the community, including crisis support as needed for ongoing support.
  • Works collaboratively with RAP staff/supervisors and managers to develop, implement, and monitor case plans
  • Completes administrative tasks, maintains effective recording/tracking systems, and responds to queries
  • Works in partnership with professionals from other organizations that support clients such as social care, health, police, education, youth-serving agencies, and early childhood services
  • Evaluate the program on an ongoing basis and recommend service delivery changes.
  • Adhere to Wesley’s policies and procedures. Attend staff meetings and training sessions as required.
  • Provide clients with education on self-care strategies, coping mechanisms, and available community health resources.
  • Maintain required participation records and relevant program statistics to update through ICARE, continuously looking for efficiencies to implement within the program.
  • Other duties as assigned.
