Responsibilities: * Assess, plan, and evaluate individualized care for residents regularly. * Coordinate resident assessments and service plans. * Liaise with healthcare professionals and maintain accurate records. * Respond to health emergencies and ensure compliance with safety policies. * Coach Care Services team members. Qualifications: * RN or RPN designation with current registration. * CPR & First Aid Certification. * Knowledge of seniors’ retirement housing and relevant legislation. * Strong communication and organizational skills. The Health Navigator is responsible for assessing, planning, and evaluating individualized care for residents while coordinating service plans. They also liaise with healthcare professionals, respond to health emergencies, and coach the Care Services team.
