What you will be doing:
In this role, you will serve as a division expert on Care Coordination within the Home and Community-Based Waiver system. You will work as a liaison for independent care coordinators and care coordination agencies and address a variety of training and support needs.

  A Bachelor’s degree from an accredited college in biological, health or behavioral science; health practice; education; public, healthcare, or business administration; or a closely related field;
