The Community Health Promotions Program aims to help patients and patients’ caregivers address and prioritize their social needs in primary care outpatient practice sites. The members of our team would like to achieve the following objectives: connect patients to community resources; inform care team members of patients’ needs as identified by patients upon the completion of screening tools and support social determinant and clinical screening processes in a variety of clinical settings. Health Promotion Advocate - Community Health Promotions Program - Days
The Health Promotion Advocate works within primary care practices, in collaboration with community-based organizations, to support patient screenings and follow-ups and connects patients to community-based resources. Provides program volunteers with assistance in delivering on-site patient education and support. Advocates are responsible for carrying out the implementation and administrative duties for the assigned program and site.
