About the role The Health Promotion Practitioner position is responsible for the planning, evaluation and delivery of primary prevention projects across the Yarra Ranges. The Grade 2 Health Promotion Practitioner reports to the Senior Health Promotion Practitioner and works with a small team planning, delivering and evaluating the strategies outlined in Eastern Health’s annual Health Promotion action plan. The Grade 2 position also has additional responsibilities such as supervising junior staff members or students as directed and independently managing your own portfolio and work with a broad range of stakeholders and partners both internal to Eastern Health and external. Due to the nature of the role working within the community and collaborative team all work will be undertaken onsite About you To be successful in this role you will possess: A tertiary qualification in Health Promotion, Public Health or another relevant qualification Advanced skills in the development, implementation and evaluation of health promotion initiatives Strong understanding of the social model of health, the determinants of health and their impact on communities and populations Strong written/verbal communication skills The ability to work independently and as part of a diverse team Please refer to the Position Description attached for further details about the role.
