Cascade AIDS Project is excited to announce that we are hiring skilled and team-oriented Health-Related Social Needs Case Manager . Cascade AIDS Project (CAP) is contracting with local CCOs and Oregon Health Authority (OHA) to be able to offer housing, outreach, and climate benefits available through this waiver to the community. The HRSN Case Manager we are hiring to do this work will be a member of CAP’s Housing & Support Services department and will work with staff, clients, and management to provide high-quality, coordinated, strengths-based social services, consistent with the agency’s mission, to the communities we serve. The HRSN Case Manager plays a key role in assisting individuals and families whose unmet social needs affect their health and overall stability. The individual will primarily work with clients referred to us from our local CCOs and OHA for rent and utility assistance but will also serve clients, as needed, with HRSN Outreach and Engagement by identifying eligible clients and assisting them with requesting HRSN benefits. This position provides client engagement, case management, and coordination of services addressing barriers such as housing, utilities, and other health-related social needs. This is a Full-Time (32 hours) role working at our Davis St. location in Old Town. The schedule for this role is Monday-Thursday 8:30am-5:00pm with three days in office and one day work from home. Starting pay is $48,547.20. The Health-Related Social Needs Case Manager's Pay increases with tenure and the top of the pay scale is $56,555. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained. Who You Are Bachelor’s degree in human/social services field (social work, public or community health, psychology) or related field plus two years of relevant experience OR four years’ experience relevant to the position Prior experience providing case management (or similar) services Demonstrated computer proficiency using Microsoft Office software (Word, Excel, Outlook) and working knowledge of the internet Excellent written and oral communication skills. Successful experience working with ethnic, racial, economic and sexually diverse populations and persons who have experienced homelessness, persons with a mental illness and/or substance addiction Demonstrated ability to effectively collaborate with community stakeholders What You’ll Do Provide case management to clients referred for HRSN rental and utility assistance, facilitating the delivery of covered services using a strengths-based model that promotes client self-determination and independence. Conduct comprehensive intake assessments to identify individual HRSN needs, including rental assistance, utility assistance, and other determinants of health. Accept and manage 3–5 new client referrals per week, incorporating them into an active caseload while ensuring timely engagement and support. Develop individualized service plans in collaboration with clients, discerning between when to provide urgent, short-term case management and when more involved assistance is appropriate. Collect and review documentation such as utility bills, rent ledgers, W-9s, and ROIs to assess appropriate use of authorized assistance. Apply complex eligibility requirements and critical thinking to determine the best course of action for client support, documenting decisions within program guidelines.
