Salary USD $26.44/Hr. Description and Responsibilities The Health Related Social Needs (HRSN) Program works in collaboration with MassHealth to address the immediate daily necessities that arise from the inequities caused by the social determinants of health, such as lack of access to basic resources including stable housing, a toxin-free environment, healthy food, heating and Internet access, transportation, healthcare, safety, education, employment, and social connection. The Housing Team Lead operates within the philosophy of the Housing First Model to assist individuals in meeting their housing needs and preferences. They assist in facilitating housing searches, acquisition of permanent housing, and work as an advocate with vocational programs, employers, therapists, doctors, and social security to support persons served as needed. The Housing Team Lead is also responsible for hiring, supervising, and mentoring the Housing Specialists within their team. Other Key Responsibilities: Assist in the development and accomplishment of program goals and objectives. Gather data, prepare, and present reports as needed. Attend, participate in, or facilitate meetings, trainings, and supervision as assigned. Participate in on-call coverage. Provide regular support, counseling and outreach to individuals and families. Attend or facilitate individual, family or group meetings as assigned. Assess life skill and healthcare needs and assist in the coordination and provision of skills training and referral for healthcare services. Qualifications High School Diploma, or equivalent required. Bachelor’s Degree in Social Services, preferred. Minimum of 2 years experience with behavioral health and/or housing, required. Valid Driver’s License and acceptable driving record, required.
