Description Why This Role Matters As a Health Services Assistant, you are a critical link between individuals we support, healthcare providers, and our clinical teams. Your work ensures medical appointments run smoothly, documentation is accurate, and health information is communicated clearly and promptly. What You’ll Be Doing Your responsibilities will include: Live our values every day by providing respectful, person-centered support aligned with our agency’s mission Coordinate medical transportation, ensuring individuals arrive safely and on time to appointments Confirm medical appointments at least one week in advance, including time, location, and accompanying residential staff Prepare medical consultation forms ahead of appointments using CaraSolva and ensure provider signatures are obtained during visits Complete and follow up on MAOS (Medical Appointment Outcome Sheets) after every appointment Communicate critical health updates promptly to the Pharmacy & Clinical Operations Administrator, nursing staff, deputies, and Health Services Director—especially when medications or treatments change Support continuity of care by emailing completed MAOS and consult forms to nursing staff, following up by phone when needed Ensure proper documentation flow by placing completed forms in the nurse’s mailbox at the individual’s residence Schedule follow-up appointments on the spot when possible—typically at three-month intervals—while the individual is still at the provider’s office Keep everyone informed by documenting all new appointments on the residence house calendar Pitch in where needed by completing other duties as assigned by the Pharmacy & Clinical Operations Administrator Requirements Minimum Qualifications: Experience working in an OPWDD-certified residential and/or day program Ability to obtain and maintain medication administration, diabetes, and G-tube certifications Valid New York State driver’s license Reliable transportation Strong organizational skills, attention to detail, and clear communication abilities Preferred Skills: Familiarity with the CaraSolva Electronic Medical Record (EMR) system Experience coordinating medical care or healthcare documentation Knowledge of OPWDD regulations and healthcare compliance Physical Requirements Ability to lift up to 50 pounds Ability to travel between residences and medical facilities Valid NYS driver’s license and access to reliable transportation
