Requirements The Health and Wellness Coordinator supports members in the Behavioral Health system under the direction of the Special Programs Manager, focusing on achieving The Arizona Vision. Key responsibilities include empowering members to manage their health, assisting with wellness goals, and delivering tailored interventions. Collaboration with care providers and community resources is crucial, involving limited travel as needed. The role emphasizes adherence to the 12 Arizona Principles, guiding service delivery through collaboration, functional outcomes, and cultural sensitivity. Essential qualifications include a passion for health, proficiency in preventive health strategies, and compliance with confidentiality standards. Strong communication, organizational skills, and basic computer literacy are also required. Physically, the role involves being stationary 50% of the time and leading health activities 60% of the time, with occasional office movement and communication with stakeholders. Overall, the Coordinator plays a pivotal role in enhancing member well-being through proactive support and collaborative efforts within the Behavioral Health system.
