SUMMARY OF DUTIES Assists the qualified nurses in providing care to all patients within an assigned unit of the hospital and such nursing care is an integral part of the individualized holistic patient care plan as directed by the Medical staff according to the Policies and Procedures of the hospital and in keeping with the generally accepted principles, policies and procedures of the Nursing Services. MAIN DUTIES RESPONSIBILITIES 1.Understands and carries out the nursing care in line with Vision and Mission Statements, Philosophy, Goals and Objectives that suit the scope of the Nursing Services and hospital. 2.Understands the service organization structure. 3.Informs Nurse-In-Charge or Unit Manager when changes occur in circumstances which affect standards of care. 4.Receives ward report and assignment before the change of each shift. 5.Attends meetings held by the services in order to discuss issues and matters pertaining to the operation of the service. 6.Daily routine: Performs regular nursing rounds on all patients in the wards to ascertain for other additional needs, talks to patient/family members. Makes and tidies patients’ beds. Change linen when necessary Carbolizes beds and equipment and tidies patient’s bedside lockers, cardiac tables, chairs and surrounding areas. Maintains the cleanliness of the ward, pantry, treatment room, utility and linen room. Collects, cleans and fills hot water flasks for patients. Dispatching: charge tickets, CSSS items, specimens, etc. to relevant services 7.Answers phone calls and relay messages. 8.Responds to patient’s call bell promptly. 9.Assists the qualified staff in the following: Carries out basic nursing care for patient’s comfort, safety, nutritional and elimination needs. Admission, transfer and discharge of patients Gives bed bath and provide total personal hygiene to non-ambulatory patients. Assist ambulatory patients when necessary. While doing dressing, setting intravenous lines, catheterization, vulva swabbing etc. Clears the trolley after use. Washes instruments used and place in the appropriate place. Recording daily charges for disposable/ equipment / consumable items used in the ward into patient’s charges accordingly. Maintaining record of intake and output chart. Lifting, turning and positioning of patients in bed, bed to chair and bed to trolley using the correct body mechanics. Treatment of pressure sores. Taking diet orders for new admission and serving meals and drinks, collects trays. Helps to feed patients orally. Prepares drink for patients at night and when necessary. Chaperons Consultant in doing procedure and ward rounds in the absence of trained nurses. 10.Accompanies the nurses to send patient to OT and from OT. 11.Escorts patients to other services for diagnostic investigations. ADDITIONAL DUTIES AND RESPONSIBILITIES IN THE SPECIFIC AREAS IN HAEMODIALYSIS SERVICE 1.Tidies the unit before the arrival of patients. 2.Assists qualified nurses in nursing procedures like cannulation, removing of cannula / AVF Needle and dressing. 3.Takes diet orders and coordinates with kitchen for meals to be supplied when necessary. 4.Prepares drinks for patients and collects cups and meal trays after used. 5.Assists in keeping the patients comfortable by giving them wet towel to freshen up, blankets if they are cold and positioned them. 6.Keeps patients cheerful and occupied by getting them newspapers and magazines to read, by switching on the TV / radio. 7.Assists patients who need to open bowel or urinate. 8.Washes and cleans bedpans and urinals. 9.Dispatches specimens to the laboratory and collects results. 10.Collects drugs and supplies from pharmacy and store. 11.Assists to wheel patients back to their cars after dialysis. 12.Disinfects dialysis machines, chairs and tables after used. 13.Cleans and tidies the store and fridge. 14.Assist in washing the dialyzer and rinsing dialyzer after and before treatment. 15.Assist in doing the priming of the dialysis circuit before treatment. 16.Assist in terminating the circuit during completion of treatment QUALIFICATIONS REQUIRED Posses academic qualification of SPM qualification. Certificate in Care Assistant. Able to converse fluently and write in English language and Bahasa Malaysia. EXPERIENCE REQUIRED On the job training/in-service training will be provided. Experience working in hospital setting is an advantage. Have interpersonal and communication skills. Caring, honest and responsible worker. Experience working in hospital is an advantage.
