The role

• Support adults or children with complex care needs, following individual care plans.
• Assist with personal care, mobility, nutrition and daily routines.
• Encourage independence and participation in activities at home and in the community.
• Carry out agreed clinical care tasks once trained and assessed as competent.
• Monitor wellbeing, maintain accurate records and report changes or concerns.
• Work closely with families and the clinical team.
• Follow safeguarding procedures for adults and children.
• Work on rota/shifts
