SURREY DOWNS HEALTH & CARE

An opportunity has arisen to join the End of Life Care team as a Band 3 Healthcare Assistant on a part-time (13 hours/week), permanent basis.
The successful candidate would be welcomed into an established team who work closely with members of the wider multidisciplinary team across all the PCN’s in Surrey Downs Health and Care.

Responsibilities

MAIN DUTIES OF THE JOB

We are looking to recruit a Healthcare Support Worker to work within the End of Life Care Team. In this role you will provide care and support to palliative patients within the community setting. You will provide a seamless service whilst a complete package of care is put in place. The post holder will undertake a range of delegated tasks. This role will be supported to develop skills and abilities to work with this client group to provide the appropriate level of care.
You will have excellent organisational and time management skills and be passionate about delivering holistic care that has the patient at it’s centre. You will relish developing new ways of working to improve quality and develop a “one team” culture. ADMINISTRATION AND CLERICAL DUTIES:

  • Maintain clear and comprehensive, signed and contemporaneous records according to Surrey Downs Health and Care procedures
  • Take and record telephone messages, respond and liaise appropriately
  • Manage stationery and documentation supplies
  • To be responsible for maintaining data entry and other record keeping e.g. recording mileage and annual leave on staff databases

Team Working:

  • Work as part of the team participating in procedures and duties in accordance with agreed standards relevant to role
  • Assist with clerical and administrative duties as required
  • Maintain regular contact with the senior team member and communicate all relevant issues

OTHER DUTIES

  • Work in partnership with patients ensuring privacy and dignity is maintained
  • Maintain close liaison with the senior team member who is responsible for the workload
  • Work in co-operation with all other professionals and agencies involved in the care of the client
  • Be aware of and act upon, where necessary, the procedures in place to protect vulnerable individuals
  • Participate in the Surrey Downs Health and Care Governance Strategy
  • Participate in Surrey Downs Health and Care Risk Management Strategy
  • Undertake such duties as may be reasonably required from time to time as are consistent with the responsibility and scale of the post
  • The postholder is employed to work within Surrey Downs Health and Care localities and may be reasonably requested to move base temporarily or on a more permanent basis, as requested by service needs
