Job Title: Healthcare Coordinator
Reporting to: Residential Director
FLSA Status: Exempt
Job Summary:
The Healthcare Coordinator-RN position carries with it the responsibilities of overseeing all aspects of healthcare for individuals in Angelwood residential programs plus training and consultation in all Angelwood programs.

Essential Duties and Responsibilities:

  • Assure Compliance with all local, state and federal regulations regarding healthcare related issues.
  • Work closely with the program supervisors to assure coordination of health care needs.
  • Obtain and document verbal orders and clarify physician orders as necessary regarding health care related issues.
  • Review consult sheets of medical appointments within 24-36 hours of the visit and communicate with the program to be sure all orders are being followed appropriately.
  • Assure compliance with Angelwood Policy of Medication Administration including:
  • Is or can become an APD approved Medication Administration Course trainer and maintain that certification
  • Training of employees in medication administration, both validation course and recertification classes as necessary
  • Plan and schedule Med Administration and recertification classes
  • Keeping track of employee strike log concerning medication errors
  • Working with program staff to provide opportunities for resolution of habitual medication errors.
  • Validate/Re-validate employees you deem competent to pass medication for those in Angelwood programs.
  • Train employees on other health care related procedures that may include but is not limited to Passive Range of Motion, Nebulizer Treatments, etc.
  • Collaborate with Program Directors regarding the pharmacy to include but not limited to:
  • Finding resolution to issues related to prescriptions.
  • Finding resolution when alternative medications must be sought
  • Conduct Medical Quality Assurance (MQA) according to the most current Angelwood MQA check list.
  • Assure that each program is completing weekly Medical Quality Assurance
  • Check first aid supplies in each program monthly as part of Medical Quality Assurance.
  • Complete at least 3 Medical Quality Assurance Audits monthly for group homes so that at least every other month, each group home has medical oversight.
  • Attend hospital discharge meetings and all other meetings deemed necessary as requested. These may include but are not limited to Physician appointments or Progress Review meetings. These will be coordinated with program staff.
  • Confer with Program Directors on Support Plan meetings and attend as necessary
  • Attend annual licensing visits to coordinate medical records and assist the APD nurse with chart and medication reviews
  • Represent Angelwood to the Agency for Persons with Disabilities, APD, at meetings related to health care, including attendance atmeetings called by the APD nurse
  • Assist Program Directors with medical appointments whenever possible if necessary.
  • Maintain proper records, to include but not limited to annual reports, monthly billing needs, medical justification statements, nursing care plans, etc. as needed.
  • Set up Medication Administration Records (MAR) for each consumer monthly and update if medications change throughout the month using the current pharmacy approved method. Each month it must be checked, signed/initialed by the HCC that the MAR matches with actual medications and doctors’ orders.
  • In challenging situations, research and advocate for best practice solutions for the health care of those in our programs. This may be related to medications or other interventions to be discussed with medical doctors as necessary.
  • Be a primary person to interface with other professionals regarding the health or medications of our consumers.
  • Attend Angelwood staff meetings as requested or necessary. This may include periodically attending monthly program meetings as needed to discuss medication/health related issues.
  • Train staff on proper use and disposal of PPE.
  • Provide training to staff and individuals served on prevention of contagious illnesses.
  • Communicate with CEO and Senior Program Director on Covid related illnesses in the agency.
  • Serve as resource for Covid or other community health concerns.

Education and Experience Requirements:

  • Current State of Florida RN License
  • Be at least 23 years of age
  • Have a valid Florida Driver’s license (or by home state if Military Family)
  • Pass Level 2 Background screening
