Overview Healthcare Coordinator Pay: $26.44 Sign-On Bonus: $2,500 Seven Hills Rhode Island is seeking a highly organized and detail-oriented Healthcare Coordinator to support the delivery and coordination of healthcare services for individuals in our Adult Services programs. This role is essential in ensuring continuity of care, effective communication between providers and teams, and the smooth operation of health-related administrative functions. The Healthcare Coordinator works closely with nursing staff, healthcare providers, program teams, and families to coordinate medical services and support the overall health and well-being of individuals served. This position combines healthcare coordination, administrative support, and direct interaction with individuals, playing a vital role in maintaining accurate records, scheduling care, and ensuring timely access to medical services. Responsibilities Care Coordination & Communication Schedule and coordinate medical, behavioral health, laboratory, and imaging appointments Serve as a liaison between healthcare providers, staff, families, and individuals supported Transport individuals to and from appointments and assist during visits as needed Communicate medical needs and basic health information to providers as directed by nursing staff Support coordination of therapies, durable medical equipment (DME), and follow-up care Administrative & Documentation Support Maintain accurate and up-to-date medical records, including appointments, prescriptions, and health documentation Assist nursing and clinical teams with recordkeeping and compliance requirements Manage medication refills, deliveries, and related coordination tasks Order and maintain medical, first aid, and emergency supplies Complete monthly safety and supply checks Team Support & Oversight Supervise Health Services Assistant(s) and provide training, coaching, and performance feedback Support staff with scheduling processes and appointment coordination Collaborate closely with nursing staff and escalate concerns as appropriate Participate in team meetings and contribute to care planning discussions Health & Safety Responsibilities Recognize and respond to medical concerns or emergencies, escalating to nursing staff as needed Follow physician orders and support proper medication and treatment procedures when applicable Promote infection control and safe healthcare practices Maintain compliance with Human Rights regulations, HIPAA standards, and organizational policies Core Competencies Strong verbal and written communication skills Knowledge of medical terminology and basic healthcare practices Excellent organizational and time management abilities Ability to work independently and manage multiple priorities Proficiency in basic computer applications (e.g., MS Word, Excel) Commitment to confidentiality, professionalism, and person-centered care Qualifications High school diploma, GED, or HiSET required Certified Medical Assistant (CMA) required 1–3 years of related experience in healthcare or human services Minimum of 2 years’ experience in developmental disabilities and 1 year in healthcare Valid Class C or Class D driver’s license with reliable, insured transportation
