Description Summary Maple City Health Care Center (MCHCC) is a Federally Qualified Health Center dedicated to improving our community’s health by making quality comprehensive healthcare accessible to all. We strive to care for others as they want to be cared for. We are seeking a dynamic and experienced individual to join our team. The Health Care Navigator contributes to the mission and values of Maple City Health Care Center by supporting patients and non-patients in accessing affordable health care. Duties and Responsibilities Assists patients and non-patients in accessing insurance information and coverage through Medicaid and the Marketplace Maintains certification as an Indiana Navigator Helps patients access in-house affordability resources Helps patients access health care financial assistance programs through Goshen Health Participates in community events to educate people about MCHCC’s navigator services Maintains a high level of accuracy and integrity with documentation Documents patient contact using electronic medical record Utilizes telephone encounters and encounter notes for documentation Follows up with care team as needed Other duties that support our shared work, as assigned Requirements Required Skills and Abilities: Experience working with diverse groups of people Ability to take initiative, work independently, and collaborate with the team Ability to respectfully translate a variety of real-life situations into the Medicaid framework Fluently bilingual: English/Spanish; oral and written Effective interpersonal skills Education and Experience: Associate degree strongly preferred, especially in social work or related field Certification as an Indiana Navigator Requirements: Ability to perform prolonged periods of time in front of a computer Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this work We are required by federal law to verify identity and eligibility to work in U.S. What We Offer: Competitive salary and benefits package (Retirement plan, health insurance, childcare reimbursement) Opportunities for professional development and growth A supportive and inclusive workplace culture The chance to make a meaningful impact on the healthcare experience in our community How to Apply: To apply for this position, please submit your resume and a cover letter that highlights your experience and how your unique background will contribute to our team.
